Maybe interview him and find out what his specific goal/need is first.

Prescribe smaller amounts and see patient more often

I think ending his Rx would be detrimental. Need to get more information

Ask him what the medication does for him? (The golden question)

You can also educate him on addiction to Rx medications and see where he stands after the discussion. He may not even realize he is slipping into addiction. Is he interested in adjunct therapies?

False negs are why I always want a pill count at the same time as the UDS collection!
